COVID-19 (Coronavirus) & Fit (Sick) Notes
Given the current position with COVID-19 and government advice to stay at home unless essential we have put a process in place a new text system to securely send sick/fit notes directly to patients. Please ensure you put your correct email address and phone number when completing the form and following assessment by a GP we will send you a text message on how to download it, which will avoid the need to attend the surgery and you can send a copy via email to your employer.
Please Note: We will not issue Fit (Sick) Notes for self isolation as there is no requirement.
- For patients who have COVID-19 or are advised to self-isolate you can obtain a "isolate note" by visiting NHS 111 online and completing the relevant form.
- For COVID-19 cases this replaces the usual need to provide a "fit note" after seven days of sickness absence.
- Isolation notes will also be accepted by Jobcentre Plus as evidence of your inability to attend.
Self Certification Fit Note for less than 7 days
If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self-Certification form yourself. Many employers have their own self-certification forms. If your employer doesn’t have its own form you can complete the Self Certification Form online.
Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.
Request a Fit/Sick note for more than 7 days
If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates.
The doctor may call you, or arrange for an appointment.
If you have already been seen by your Doctor about this illness your certificate will be sent to you via secure text message.
